Please carefully follow these printing instructions:

> Ideally, use a laser printer to print. The document must be printed exactly, with the front (page 1) and back (page 2) identical to the pdf on one sheet of paper. It must be printed double-sided-- and the reverse side cannot be “upside down.”

> The document must be printed at 100% or “Actual” size, not shrunk to 80% or any other number.

> The document cannot be two one-sided papers taped or stapled together.

> We recommend you print a few copies of page 1, and then place them back into your printer, and then print page 2 until its oriented perfectly. Make sure the document doesn’t end up with page 2 “upside down.” If it does, print another copy.

> No colored paper! It should be printed on plain, white paper— NOT yellow paper, card stock, etc.
